Home School Registration

Utah State Law requires that you file a signed and notarized affidavit with the school district of residence to Home School your child(ren). (Utah Code 53G-6-201, 53G-6-204). To home school your child(ren), please complete and notarize the Home School Affidavit and return the form to Canyons School District Student Enrollment office.

If you will no longer be home schooling your child(ren); please complete the Home School Non-Renewal form and return it to Canyons School District Student Enrollment office.

Exemption Process

  1. A parent must register their student for home school prior to being withdrawn from their public boundary school.
    1. Complete and notarize the Home School Affidavit.
      1. Pick up the form at the district office located at 9361 South 300 East, Sandy, UT 84070 or
      2. Print out form online HERE.
    2. Mail or deliver to Canyons School District Office:
      1. Student Enrollment 9361 South 300 East, Sandy UT 84070
    3. Once the Home School Affidavit is processed by the Student Enrollment office a copy of the signed certificate of exemption will be mailed back to the parent.
  2. When the parent receives the copy of the processed home school affidavit in the mail, they may then take that processed copy of the home school affidavit to the boundary school to withdraw the student(s) from public school.

Lucie Chamberlain

Alta View Elementary

If a movie about super teachers were ever made, Lucie Chamberlain would be a prime candidate for a leading role. Fortunately for her kindergarten students at Alta View Elementary, she already thrives in a supporting role for them. Parents thank her for being a “super teacher.” She is also described as an “amazing colleague.” Whether students need help in the classroom or from home while sick, Lucie goes above and beyond to help them learn, overcome fears, and feel important and cared for. Lucie is the reason a number of kids went from hating school to loving it, according to parents. The way she exudes patience, sweetness, positive energy, and love for her students with special needs melts is appreciated and admired. One parent noted: “Both my kids wish she could be their teacher forever.” Another added:  “She treats every student like their learning and their feelings are her priority.” Super teacher, indeed!
