504 Accommodations

ADA Section 504, Subpart D
A student could have a Section 504 Accommodation Plan if he/she has been identified, and the evaluation shows that the individual has a mental or physical impairment that substantially limits one or more major life activities. This determination is made by the school team of knowledgeable individuals, including the parent(s), who are familiar with the student and his/her disability. School 504 Coordinators are the school psychologist assigned to the school. Please see the school directory for the contact person at your student’s school.
